29 CFR 1910.178(p)(1): Powered industrial truck(s) with defect(s) or in any way unsafe had not been withdrawn from service until restored to safe operating condition: Employees were exposed to injury while operating the powered industrial trucks because: a. the horn on the Yale #25341 Electric Forklift, Model Number ERC050RGN36TE082, Serial Number E108V15064Y, was not operational and the forklift was not taken out of service. b. the rear light on the Yale #11171 Veracitor 50VX Propane Forklift, Model Number GLC050VXNVSV061, Serial Number A910V20785K, was not operational and the forklift was not taken out of service. c. the lights on the Yale #11049 Veracitor 50VX Propane Forklift, Model Number GLC050VXNVSV061, Serial Number A910V19228J, were not operational and the forklift was not taken out of service. d. the front light on the Yale #11072 Veracitor 50VX Propane Forklift, Model Number GLC050VXNVSV061, Serial Number A910V19367J, was not operational and the forklift was not taken out service. e. the rear light on the Yale #11061 Veracitor 50VX Propane Forklift, Model Number GLC050VXNVSV061, Serial Number A910V19514K, was not operational and the forklift was not taken out of service. f. the propane tank was not latched on the Yale #11061 Veracitor 50VX Propane Forklift, Model Number GLC050VXNVSV061, Serial Number A910V19514K, and the forklift was not taken out of service.
