29 CFR 1910.25(d)(1)(x): Portable wood ladder(s) were not inspected frequently and those which have developed defects shall be withdrawn from service for repair or destruction and tagged or marked as "Dangerous, Do Not Use." a) First floor by box room - A damaged portable wooden ladder was not taken out of service and was not tagged exposing employees to a fall hazard of approximately eight (8) feet, on or about February 20, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
