29 CFR 1903.19(d)(1): The employer did not submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required: (a) Blizzard Ice, LLC, failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 for hazards associated with: 1) developing energy control procedures, implementing employee training and periodic inspections for lockout/tagout; 2) training forklift operators prior to allowing them to operate powered industrial trucks; 3)guarding on the main motor drive for the Hammer and Stiching section of the bagging machine; 4) guarding of the chains and sprocket drives on the filler section, end conveyor section and the main conveyor motor section of the bagging machine; and 5) a cover over the electrical panel for the bagging machine and a cover over the electrical junction panel attached to the wall next to the overhead doors.
