(a) Location: 36 River Street, Waltham, Ma On or about 1/30/14, an employee was observed using the top step of a stepladder as a step. Shawnlee Construction was previously cited for violation of this Occupational Safety and Health standard or its equivalent standard 1926.1053(b)(13), which was contained in OSHA inspection number 311592430, citation number 1, item number 004 issued on 4/20/2010 and was affirmed final order on 9/10/10 with respect to a workplace located at 195 Hanover Street, Portsmouth, NH 03801
