29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent: a) Baillie Lumber Co., L.P., Galion, Ohio: On or about December 3, 2013, the employer did not meet the basic requirements for recording injuries and illnesses. OSHA Form 300 or equivalent was not maintained for calendar year 2013. The employer had 34 employees on site and had one recordable injury in 2013.
