29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings: (a) On or about September 10, 2013 the inspection revealed that employees were exposed to serious injury from a pipe chase pit with the cover removed that was not adequately protected. The pipe chase was located in the tanner valve #4 storage room at the John Day Dam. (b) On or about September 16, 2013 the inspection revealed that employees were exposed to serious injury from pit openings that were not protected by a floor cover or guardrails. The pits was located in the level 131 fish turbine room at the John Day Dam. The Army Corps of Engineers was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 730483, citation number 1, item number 1 and was affirmed as a final order on April 24, 2013, with respect to a workplace located at The Dalles dam in the The Dalles, Oregon.
