29 CFR 1910.179(j)(2)(iv): Visual inspection daily and monthly of the overhead crane hoist chains with a certification record was not accomplished which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected: a) In the Fabrication Shop ceiling. Coffing bridge crane hoists, two 4-ton, Model ECMT 6016-4. On or about September 12, 2013, and at times prior thereto, employees were exposed to struck-by hazards of malfunctioning overhead crane hoists, from the lack of inspection certification, while welding and fabricating metal parts below. b) In the Fabrication Shop ceiling. Coffing bridge crane hoists, six 2-ton, Model EC-4016-2. On or about September 12, 2013, and at times prior thereto, employees were exposed to struck-by hazards of malfunctioning overhead crane hoists, from the lack of inspection certification, while welding and fabricating metal parts below.
