29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent. Injuries and illnesses occurred at Tri State Timber, but the injuries were not recorded on the OSHA 300 and 300A or their equivalent including a worker suffering heat stress, a worker having his hand crushed, a worker having his foot lacerated, and a worker suffering from repeated trauma to the elbow which were not recorded. Years records not kept include: (a) For the year 2012, the employer did not maintain the OSHA 300 log or it's equivalent of recordable injuries and illnesses, and did not record an employee being killed; (b) For the year 2011, the employer did not maintain the OSHA 300 log or it's equivalent of recordable injuries and illnesses, and did not record an employee being crushed under a tree; (c) For the year 2010, the employer did not maintain the OSHA 300 log or it's equivalent of recordable injuries and illnesses, and did not record an employee getting cut by a chain saw; (d) For the year 2009, the employer did not maintain the OSHA 300 log or it's equivalent of recordable injuries and illnesses; (e) For the year 2008, the employer did not maintain the OSHA 300 log or it's equivalent of recordable injuries and illnesses; and (f) For the year 2007, the employer did not maintain the OSHA 300 log or it's equivalent of recordable injuries and illnesses.
