29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident. On or about September 3, 2013 and times prior thereto, Ponder Well Service, Inc. failed to report to OSHA a work-related death of an employee at the Wepfer A1 well located at or near Latitude coordinate North 33.4035 and Longitude coordinate West 93.13903 in McNeil, Arkansas.
