29 CFR 1910.1904.29(b)(1): The log of occupational injuries and illnesses, (OSHA Form No. 300), was not completed in the detail provided in the form and instructions contained therein: (A) United Parcel Service, Inc. (UPS) @ 1 UPS Way in Hodgkins, IL: On or about December 13, 2011, the employer did not provide the required location and/or injury or illness description on the OSHA Form 300 or equivalent for the calendar year 2011. a. On or about July 18, 2011, while working in a SE Primary, an employee (#2) sustained dehydration and heat exhaustion (Case Log #107249911), which resulted in days away from work. The description recorded "Other - employee fainted while walking out of the building FROM Unknown" does not adequately describe the "object/substance that directly injured or made person ill" as required in Column F on the OSHA Form 300. b. On or about July 19, 2011, while working in a Bullfrog 8 and Outbound 8, an employee (#6) sustained dehydration, heat syncope, nausea with vomiting and heat exhaustion (Case Log #107299911), which resulted in medical treatment beyond first aid. The description recorded "other FROM The heat" does not adequately describe the injury or illness as required in Column F on the OSHA Form 300. c. On or about July 22, 2011, while working in a Outbound 8, an employee (#9) sustained heat exhaustion, chest pain & vomiting (Case Log #107318311), which resulted in days away from work. The description recorded "Not feeling good due to the heat FROM The heat" does not adequately describe the injury or illness as required in Column F on the OSHA Form 300. d. On or about July 22, 2011, while working in a Bullfrog 2, an employee (#10) sustained heat exhaustion and heat fatigue (Case Log #107340211), which resulted in days away from work. The description recorded "alleged heat related discomfort FROM unknown" does not adequately describe the injury or illness and "object/substance that directly injured or made person ill" as required in Column F on the OSHA Form 300. e. On or about July 22, 2011, while working in a Outbound 6, an employee (#11) sustained loss of consciousness and cardiac arrest (Case Log #107319011), which resulted in death. The description recorded "unknown FROM unknown" does not adequately describe the injury or illness as required in Column F on the OSHA Form 300. f. On or about July 27, 2011, while working in a Outbound 7, an employee (#12) sustained dehydration and severe heat exhaustion (Case Log #107319011), which resulted in hospitalization and days away from work. The description recorded "head Said he felt light headed FROM light headedness" does not adequately describe the injury or illness and "object/substance that directly injured or made person ill" as required in Column F on the OSHA Form 300. g. On or about July 28, 2011, while working in a NE Bulk Split Primary, an employee (#13) sustained heat exhaustion (Case Log #107393511), which resulted in medical treatment beyond first aid. The description recorded "unknown FROM unknown" does not adequately describe the injury or illness and "object/substance that directly injured or made person ill" as required in Column F on the OSHA Form 300. h. On or about August 2, 2011, while working in a SE Primary, an employee (#14) sustained heat exhaustion (Case Log #107461611), which resulted in days away from work. The description recorded "headache and felt nauseas FROM packages" does not adequately describe the injury or illness and "object/substance that directly injured or made person ill" as required in Column F on the OSHA Form 300.
