29 CFR 1904.4(a): An employer required to keep records of work-related fatalities, injuries, and illnesses did not record each new case resulting in a fatality, injury or illness in accordance with criteria listed in 1904.7-12. The employer did not maintain OSHA Injury and Illiness logs. There was at least one recordable case in July 2011. Abatement documentation is required for this item in accordance with the required of 29 CFR 1903.19(d).
