29 CFR 1910.26(c)(3)(i): A simple rule for setting up a ladder at the proper angle is to place the base a distance from the vertical wall equal to one-fourth the working length of the ladder: (a) Enterprise Products Operating, LP at 1250 County Road 69, Rifle, CO 81650: On and before 8/1/13, the employer did not ensure that the portable metal ladder in cooling fan and slug catcher was at the proper angle for ensure safe use. The extension ladder was not placed at the proper angle with a secure footing. It was braced against the underside of the cooling fan. Employees use the ladder to perform maintenance work on the fans and the slug catcher. This condition exposed employees to slip, trip, and fall hazards.
