29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment: No written workplace hazard assessment had been conducted or certified by the employer. The employees perform operations including, but not limited to, grinding metal parts, operating milling machines, and using compressed air. NOTE: The assessment shall be written, titled, dated and certified by signature. The required documented training shall include the following: 1) When PPE is necessary; 2) What PPE is necessary; 3) Limitations of the PPE; 4) How to properly wear and use PPE; and, 5) Inspection and care of the PPE
