29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Building Number One; Press Brake Areas - The TrumaBend V130 press brake did not have point of operation guarding. The operator used the foot pedal to cycle the press instead of the two-hand control mechanism. When a helper was needed to bend product, only one of the employees had control of the foot pedal, on or about May 2, 2013. (b) Building Number One; Press Brake Areas - The Cincinnati 175 ton press brake did not have point of operation guarding. The operator used the foot pedal to cycle the press instead of the two-hand control mechanism. When a helper was needed to bend product, only one of the employees had control of the foot pedal, on or about May 9, 2013. (c) Building Number Two; Press Brake Areas - The TrumaBend C120 press brake did not have point of operation guarding. The operator used the foot pedal to cycle the press instead of the two-hand control mechanism, on or about May 9, 2013. THE PRECISION CUT INDUSTRIES, INCORPORATED WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.212(A)(3)(II), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314239070, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON MAY 21, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 115 RAM DRIVE, HANOVER, PENNSYLVANIA, 17331. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
