Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Tradewinds Resort, Jacaranda Beach Villas South Elevator - Fall protection was not provided for employees removing oily water and rags from an elevator shaft. The shaft was 6 feet 5 1/4 inches deep from the top of the outside opening of the hoistway, on or about April 24, 2013. SWS Environmental Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.132(a), which was contained in OSHA inspection number 313781205, citation number 1, item number 1B and was affirmed as a final order on April 8, 2010, with respect to a workplace located at 7300 St. Hwy 199, Upper Sandusky, OH 43351.
