29 CFR 1904.39(a): Within eight (8) hours after the death of any employee from a work-related incident as a result of a work-related incident, you did not orally report the fatality by telephone or in person to the Area Office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor, that is nearest to the site of the incident: a) 14 Bedford Street, Dewey Beach, DE (Indian Beach) - The employer did not report a death of an employee. On January 23, 2013, an employee fell to his death from an unguarded 3rd story deck, approximately 20-25 feet above the ground. The employee fell off the deck while he used a ladder to install deck joists and posts at the residential construction site. The violation was observed on or about March 6, 2013.
