29 CFR 1904.4(a): The employer does not record on its OSHA 300 Log each work-related fatality, injury or illness that meets the general recording criteria. At The Shaw Group, Inc. DBA Shaw Modular Solutions, the employer did not record the following workplace injuries on the OSHA 300 Log for calendar year 2013: a) The employer does not ensure that each work related fatality, injury or illness that meets the general recording criteria is recorded on the OSHA 300 log. This violation occurred on or about January 25, 2013, when employee SMS-13-001 suffered a left knee injury resulting in medical treatment beyond first aid when an immobilizer and prescription drug was given to the employee. The employee was injured while turning suddenly at work.
