29 CFR 1904.29(a): A Log of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injuries and Illnesses, (OSHA Form 300-A), and/or the Injury and Illness Incident Report (OSHA Form 301), or equivalent forms were not kept by the establishment: a) The Mohawk Company dba Metal Coating Company, Lima, Ohio: On or about June 13, 2011, the employer did not meet the basic requirements for recording injuries and illnesses. OSHA forms 300 and/or 300A were not maintained for calendar year 2011. The company had approximately 30 employees and had 2 recordable injuries.
