29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about 11/06/2012, at 2409 N. Ocean Blvd. Gulfstream, Fl. an employee was working from an bucket truck lift without being attached to the basket. The employee was exposed to a fall hazard of approximately 20 feet.
