29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) 5th floor, south side, elevator shaft: An employee was instructed to fill concrete into formwork pockets at the edge of the building and adjacent to an open elevator shaft. To access his work area, the employee removed a section of the guardrails around the elevator shaft and laid a wood plank across the shaft opening to walk across. The employee did not have fall protection and was not protected from falling into the shaft opening when he removed the guardrail for the elevator shaft to work. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATIONS AS NORMALLY REQUIRED BY 29 CFR 1903.19.
