29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a) Morning Star Fellowship Church - On or about July 16, 2012, employees are using the scaffold components to access the scaffold platform, no less than 14 feet high, and a ladder is not provided for safe access. JOHN MINDLER PLASTERING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312973225, CITATION NUMBER 1, ITEM NUMBER 1d, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 6, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT 1446 W.
