29 CFR 1926.251(d)(6): Natural and synthetic fiber rope slings were not immediately removed from service when any of the following conditions specified in 1926.251(d)(6)(i) through (d)(6)(vi) were present: a) Thunderbolt Builders, Inc. at 416 Wind River Drive, Windsor, CO 80550: On or before June 9, 2012, the employer provided a defective synthetic fiber rope sling for use in lifting bundles of 2 x 4's to the second floor. Condition exposed employees to struck-by hazards.
