29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective: Specialty Packing Division: On or about May 30, 2012, path to ground on the AMD HV-60KVWP Scale #5 was not permanent, continuous and effective. The metal-encased scale was missing a ground pin. C & S Wholesale Grocers, Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection Number 310189519, Repeat Citation Number 2, Item Number 3, issued on July 28, 2008, with respect to a workplace located at 500 North Street, Windsor Locks, Conecticut.
