During the walkaround with William Pierson, EHS Manager, Jefferey Klunk, Environmental Manager, and William Johnson, Operations Manager, I noted that one or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a/ Inside Building one, Post 23k, the chuck and unused portion of the bit belonging to the Cincinnati Milacron, M-41, were unguarded. William Pierson stated that he used to manage this department for 11 years prior to assuming his current duties. He added that b/ Inside Building One, Post 23K, the chuck and unused portion of the bit belonging to the Powermatic Drill Press, m/n 1150A, s/n 9015V341, were unguarded. c/ Inside of the Maintenance Building, the chuck and unused portion of the bit belonging to a Clausing Drill Press, m/n 2287, s/n 252092, were unguarded. d/ Inside of the Maintenance Building, the chuck and unused portion of the bit belonging to a Bridgeport Milling Machine, 108100, were unguarded. e/ Inside of the Maintenance Building, the chuck belonging to a Clausing Colchester VS13 Lathe was unguarded. f/ Inside of the Maintenance Building, the chuck and unused portion of the bit belonging to a Southbend/BARMIA Drill Press, s/n 1677L, were unguarded. g/ Inside of the Maintenance Building, the chuck and unused portion of the bit belonging to a Bridgeport Milling Machine, 100378, were unguarded. h/ Inside of Building Two, Post DB-3, the chuck and unused portion of the bit belonging to a Alliant Milling Machine, m/n Alliant-RT-2, s/n 80612736, were unguarded. i/ Inside of Building Two, Sheetmetal Area, Post IN-5, the chuck and unused portion of the bit belonging to a Rambaldi Milling Machine, m/n FJ096431, were unguarded. j/ Inside Building Two, Composites, Maintenance Shop, the chuck and unused portion of the bit belonging to a Bridgeport Milling Machine, s/n 11385812, were unguarded. k/ Inside Building Two, Composites, Maintenance Shop, the six chucks and unused portion of the drill bits belonging to a Burgmaster Six Spindle, m/n 2A, s/n 254, were unguarded. l/ Inside of Building Two, Composites, Maintenance Shop, the chuck and unused portion of the bit belonging to a Jet Drill Press, unknown m/n or s/n, were unguarded. m/ Inside of Building Two, Composites, Maintenance Shop, the chuck belonging to the Clausing Colchester VS-13-Inch Lathe was unguarded. n/ Inside of the Hard Metal Area, the chuck and unused portion of the bit belonging to a Cincinnati Milacron, CI5X-M2, were unguarded. o/ Inside of the Sheetmetal/Tooling/Maintenance/Repair Department, the chuck and unused portion of the bit belonging to a Bridgeport Milling Machine, s/n 77538-2, were unguarded. p/ Inside of the Sheetmetal/Tooling/Maintenance/Repair Department, the chuck and unused portion of the bit belonging to a Bridgeport Textron, 145168, were unguarded. q/ Inside of the Sheetmetal/Tooling/Maintenance/Repair Department, the chuck and unused portion of the bit belonging to a Webb Milling Machine, 136886, were unguarded. r/ Inside of the Sheetmetal/Tooling/Maintenance/Repair Department, the chuck and unused portion of the bit belonging to the Cincinnati Bickford Radial Arm Milling Machine, 003921, were unguarded. s/ Inside of the Sheetmetal/Tooling/Maintenance/Repair Department, the chuck and unused portion of the bit belonging to the Alzmetall Milling Machine, 134509, were unguarded. t/ Inside of the Sheetmetal/Tooling/Maintenance/Repair Department, the chuck belonging to a Monarch Machine Tool Co. Lathe, 805380, was unguarded. u/ Inside of the Sheetmetal/Tooling/Maintenance/Repair Department, the chuck belonging to a Hendley Machine Co. Lathe, 890009, was unguarded. GKN Aerospace was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.212(a)(1) which was contained in OSHA Inspection 311761100, citation 1, item 4, which was issued on 08/11/09.
