29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and procedures; and, changes to facilities that affect a covered process: a- Hess Port Reading Refinery, Dimersol - On 04/23/10 a frac tank being used during the final water wash and drying of the R-2203 caught fire and ruptured. At the time of the incident, tank contents included wash water, sludge and hydrocarbon. MOC 10-1462-22 (waterwash R-2203 and dryout prior to startup) approved on 02/03/10 failed to address build up of flammable vapors in the tank and potential for static electricity. In addition, the MOC did not address the installation of a spill guard berm under the frac tank along with proper grounding and bonding. Hess Port Reading Refinery was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.119(l)(1), which was contained in OSHA Inspection number 311917009, Citation 1, Item 25, issued on 05/13/09, with respect to a workplace located at 750 Cliff Road, Port Reading, NJ 07064. The citation became a final order on 12/24/09.
