Text For Citation: 02 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls and crushing hazards: (a) For the employees being hoisted by an Ingersoll Rand pneumatic winch, model FA2.5A-13MX1, serial number MW006719, on Nabors Drilling rig #460 located in McKenzie County, North Dakota. The winch was not designed or approved by the manufacturer for hoisting or lowering personnel. Note: One feasible and acceptable method of abatement would be for the employer to replace the pneumatic winch with an approved hoist and provide specific training and policy prohibiting the use of unapproved devices for hoisting personnel. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 310591268, Citation Number 1, Item Number 1, issued on December 10, 2007.
