Text For Citation: 02 Item/Group: 001 Hazard: CRUSHING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputations from unexpected machine cycles. a) Shop Press Brake Production Area - Niagra Press Brake # IB-3668. A press brake operator was removing material from the die area, when the machine unexpectedly cycled due to a solenoid failure, resulting in crushed and lacerated finger injuries, on or about, 03/25/09. In addition, periodic and regular inspections of press brakes were not conducted, on or about, 04/09/09. ONE FEASIBLE AND ACCEPTABLE METHOD OF ABATEMENT WOULD BE TO: 1) Follow the ANSI B11.3 Press Brake Standard Section 5.1.4 entitled "Inspection and Maintenance Records", which states "It shall be the responsibility of the employer to establish and follow a program of periodic and regular inspections of his power press brakes to ensure that all of their parts, auxiliary equipment, and safeguards are in safe operating condition and adjustment. The employer shall maintain records of these inspections and the maintenance work performed as required for normal business operations". A&S Associates, Inc was previously cited for a violation of this occupational safety and health standard [Section 5(a)(1) of the OSH Act], which was contained in OSHA Inspection Number 310328679, Citation Number 1 Item Number 1, and was affirmed as a final order on 09/12/2006, with respect to a workplace located at 4806 North Manhattan Avenue, Tampa, Florida 33614.
