29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation: a- Hess Port Reading Refinery - The 2008 Log maintained by the employer contained a days away case (#08-02) not recorded as required by 1904.7(b)(3) in that the injury was incorrectly classified as other recordable case with no total for days away entered. The injury description of the last entry (case #08-07) omitted the object/substance which directly injured the worker. Condition noted on or about 11/19/08. b Hess Port Reading Refinery - The 2007 Log maintained by the employer contained cases involving hand/finger fractures (#07-04 and #07-08) where the injury description omitted the object/substance which directly injured the worker. Condition noted on or about 11/19/08.
