Nevada Revised Statute 618.375(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: The employer failed to ensure that operators blocked the truck wheels when not in the operating position of a rough terrain forklift while elevating personnel, as follows: a.At Crowne Professional Park at Stephanie construction site, 50 South Stephanie Street, Henderson, NV 89012, framing activities, two employees were exposed falls while working at an elevation of approximately 20 to 25 feet when the operator of the rough terrain forklift failed to block the truck wheels when not in the operating position. Reference: ANSI/ITSDF B.56.6-2005 Safety Standard for Rough Terrain Forklift Trucks, Section 5.15, Paragraph 5.15.1, Subparagraph (s) Paragraph 5.15.1 Elevating Personnel, Subparagraph (s): When the operator is not in the operating position, block the truck wheels... One feasible means of abatement would be to require the operator of the rough terrain forklift to complete a refresher course in forklift safety.t
