GULF COAST PRE-STRESS, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD SECTION 5(a)(1) of the OCCUPATIONAL SAFETY HEALTH ACT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 308773613 CITATION NUMBER 1, ITEM NUMBER 2, ISSUED ON 5/9/2007, WITH RESPECT TO A WORKPLACE LOCATED AT 494 NORTH MARKET STREET, PASS CHRISTIAN, MS. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish employment and a place of employment which were free from recognized hazards that are causing or/and likely to cause death or serious physical harm to employees in that employees are exposed to hazard of being crushed by: (a)Gulf Coast Pre-Stress, Inc. fails to ensure that employees are not exposed to crushed by hazards from traveling under elevated loads. The practice of failing to ensure that loads are not lifted over employees was most recently observed at a worksite located at 494 North Market Street, Pass Christian, MS on January 30, 2008 at the east side of the pre-stress yard where an employee drove a front end loader under a load being moved by a Mi-Jack Travelift crane. To abate this hazard Gulf Coast Pre-Stress, Inc. must ensure that crane operators do not lift loads over employees and employees do not walk, stand, work or drive under loads. Date by which violation must be abated: Immediately upon receipt of the Citation and Notification of penalty. Pursuant to 29 CFR 1903.19, Gulf Coast Pre-Stress, Inc. within 10 days of receipt of the Citation and Notification of Penalty must submit documents describing the steps it is taking to ensure compliance, including establishing and enforcing work rules that prohibit employees from working, walking or standing under loads and prohibit crane operators from lifting loads over employees..
