Text For Citation: 02 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Location - 600 Unicorn Park, Woburn, MA: On or about 06/22/06, employees were exposed to a serious fall hazard of up to 60 feet while working at the roof edge without the use of personal fall protection equipment. (b) Location - 600 Unicorn Park, Woburn, MA: On or about 06/22/06, employees were exposed to a serious fall hazard of up to 60 feet while working from a rope descent system connected to a rolling roof system without independent anchorage points to the roof for their personal fall protection equipment. Among other means, feasible means of abatement would be to: 1) Assign a competent person to each job site and ensure that (s)he inspects the set-up, rigging, anchorage and all aspects of the outrigger suspension equipment prior to and during each use. 2) Ensure that the outrigger beam suspension unit such as the rope descent equipment is anchored or tied back to a secure independent anchorage point on the building at all times when in use as required by ANSI/IWCA STD I-14.1-2001 and the manufacturer's instructions. 3) When scheduling work to be performed, ensure that a competent person conducts a full and complete on-site inspection and hazard assessment for rigging, anchorage and operation of the outrigger suspension units to be used in the window washing operations. 4) Ensure employees are supplied with the proper fall protection equipment while on the roof and suspended by the rope descent system.
