Text For Citation: 02 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of falls in the event of cable failure. a) On or about August 22,2006, wire rope cables were not replaced at intervals recommend by the manufacturer. (1) At the Foley's (Macy) Store at the La Cantera Mall, San Antonio TX, an employee working from a 540 Wilkie Aerial ladder was exposed to the hazard of the ladder retracting and/or collapsing due to a defective cables. The two extension cables had not been replaced since Jan 2003. Among others methods, feasible and useful, one method of abatement method to correct this hazard would be to 1) inspect and replace the cable as required by the Wilkie Manfacturer's Manual which states under "Lubrication & Inspection Specifications", page 7: Inspect all steel cable for any broken strands, excess rust or corrosion or damaged areas. Replace if any of these are found in cable. 2) Install new cable as per J. Page 9 "Inspection & Lubrication Record" -"J" Install new cable every 24 months.
