Text For Citation: 02 Item/Group: 001 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to point of operation hazards where: a) The Verson U05 mechanical press brake was not equipped with anti-repeat features as required by The American National Standards Institute (ANSI) industry standard B11.3, 1982 edition, section 3.1; b) The Verson U05 mechanical press brake was not equipped with two-hand controls requiring concurrent pressure as required by ANSI B11.3, section 3.6; c) There were no inspection records conducted nor inspection record verification maintained for the Verson U05 mechanical press brake as required by ANSI B11.3, section 5.1.4; d) Operator training was not provided for employees operating the Verson U05 mechanical press brake as required by ANSI B11.3, section 6.1.2 Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure the press brake is equipped with anti-repeat and concurrent pressure features incorporated into the machine's two-hand controls; to ensure inspections of the press brake are conducted and documented; and ensure operators are adequately trained to include operation of the press brake without placing hands in the die of the press brake.
