Text For Citation: 02 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to the hazard of falling down the hoist way shaft when the elevator car was not in the landing zone, being struck by the elevator car when the car was entering or leaving the landing zone, or having part of their body caught between the moving elevator and the hoist way: (a) VERTICAL RECIPROCATING CONVEYOR LOCATED BETWEEN THE MAINTENANCE DEPT. AND THE WIRE DRAWING DEPT. ON THE FIRST LEVEL AND IN THE STORAGE AREA ON THE SECOND LEVEL -- THE GATE ON THE SECOND LEVEL COULD BE OPENED WITHOUT THE VERTICAL CONVEYOR CAR AT THE SECOND LANDING ZONE. EMPLOYEE(S) WERE NOT PREVENTED FROM WALKING INTO THE HOISTWAY ON THE SECOND FLOOR LEVEL WHILE THE VERTICAL CONVEYOR WAS ON THE FIRST LEVEL. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THE HAZARD, WOULD BE TO INTERLOCK THE EXITING ACCESS GATE AT THE SECOND FLOOR LEVEL OF THE VERTICAL CONVEYOR, WHEREBY THE HORIZONTAL GATE COULD NOT OPEN, UNLESS THE CAR OF THE VERTICAL CONVEYOR WAS AT THAT LEVEL. Riverdale Mills Corp. was previously cited for violation of this Occupational Safety and Health Standard, or it's equivalent standard, Section 5(a)(1) of the Occupational Safety and Health Act of 1970, which was contained in OSHA Inspection No. 300510153, Citation No. 01,Item No. 01, issued on 12/20/2000.
