Text For Citation: 02 Item/Group: 001 Hazard: CAUGHT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being caught in an in-running nip/pinch point on a moving belt conveyor and were unable to shut the conveyor off quickly because no operable emergency stop cord or similar device was present: On or about 11/19/03, in the Tire Recycling Area, at the # 1193 input feed conveyor for the "Barclay" tire shredding machine: While in the process of loading tires on the conveyor, an employee's leg got dragged into an in-running nip/pinch point at the "turn around roll" at the south end of the conveyor. The employee grabbed the emergency stop pull cord, but it was not operable. Among others, a feasable method to eliminate or materially reduce this hazard is to equip the conveyor with permanently installed pull cords or similar stop controllers which comply with ASME B20.1-1996, Safety Standard For Conveyors and Related Equipment, Section 5.11.2(c) and Section 6.1.2(c).
