Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following conditions: (a)Millennium/Rigid Building Systems, 18933 Aldine Westfield, Houston, TX, 77073, Bay 1, employees were exposed to the hazard of being struck by and/or crushed by steel plate because below-the-hook lifting devices such as the plate loader vacuum lift S/N 95-15-3688 attached to the underhung crane by the plasma machine, was not frequently and periodically inspected to identify hazardous deficiencies in the lifting device. Of particular concern with this crane and UTHLD is that the weight of the UTHLD and the steel plate it lifts is within a few pounds of the total capacity of the crane hoist. One feasible and useful abatement method to correct this hazard includes, but is not limited to, inspecting all below-the-hook lifting devices in accordance with ANSI B 30.20, Below-The- Hook-Lifting-Devices. MILLENNIUM/RIGID BUILDING SYSTEMS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 304512221, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON 04/03/2003.
