Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)the hazard of being struck by well tooling, pump rods, and/or portions of the rig structure of the #1 Workover Rig, located at the Edelblute Number 1 well, Powerline Road, Rosenberg, TX, following failure of the hoist line due to pre- existing damage. Among other methods, one feasible and acceptable abatement method to correct the hazard is to regularly inspect all workover rig hoist lines for evidence of damage due to abuse or excess wear, and to replace damaged lines as deemed necessary, as specified by Section 9.6.1 of the American Petroleum Institute Standard RP 54-1999, "Recommended Practice for Occupational Safety and Health for Oil and Gas Well Drilling and Servicing Operations."
