Minn. Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees; and On May 15, 2000, training and written operational procedures were not effectively written and followed for the safe operation of the feed processing equipment which lead to a fire and explosion(s) within the facility, resulting in serious injuries to employees. Operational procedures need to be reviewed, rewritten, and then training provided for employees to assure safe procedures are followed in the future before production resumes.on
