29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Located at Genpak, LLC, Columbus, Ohio, the employer did not record the following workplace injuries and illneses correctly on the OSHA 300 log for the calendar year 2010: a) On or about May 20, 2010, an employee was identified as having a Standard Threshold Shift (Case 3) and column M6, "All Other Illnesses", was marked instead of M5, "Hearing Loss". b) On or about May 21, 2010, an employee was identified as having a Standard Threshold Shift (Case 4) and column M6, "All Other Illnesses", was marked instead of M5, "Hearing Loss". c) On or about August 11, 2010, an employee fractured their right tibia (Case 5). The injury was not classified (Column G, H, I, or J) for the most serious outcome for the case. d) On or about September 2, 2010, an employee sustained an injury to their back (Case 6). The injury was classified as column I, "Job transfer or restriction", and column K indicated that the employee was away from work for 25 days. e) On or about November 15, 2010, an employee sustained an injury to their lower back (Case 7). The injury was classified as column I, "Job transfer or restriction", and column K indicated that the employee was away from work for 4 days. f) On or about March 5, 2010, an employee sustained an injury to their right hand and fingers (Case 1) and column "F" of the OSHA-300 for 2010 did not include all the information required. Column "F" must include a description of the injury, the parts of the body affected, and the object/substance that directly injured or made employee ill. g) On or about March 19, 2010, an employee sustained an injury to their right finger (Case 2) and column "F" of the OSHA-300 for 2010 did not include all the information required.
