29 CFR 1910.119(j)(4)(iii): The employers frequency of inspections and tests of process equipment was not consistent with applicable manufacturers recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience: Suncor Energy Inc, at 5801 Brighton Boulevard, Commerce City, CO: On and preceding 3/13/12, Suncor Energy Inc 's frequency of inspections and tests of process equipment was not consistent with applicable manufacturer's recommendations and good engineering practices. Tank T-400, which contained approximately 1.4 million pounds of liquefied petroleum gas (LPG), was located in Unit 41. Tank T-400 had a single temperature indicator within the refinery's Critical Alarm System (CAS). This temperature indicator, consisting of a Rosemount Inc 0078 Resistance Temperature Detector (RTD) and a Rosemount Inc 3144 Temperature Transmitter (TT), had been installed in 2006. The Resistance Temperature Detector and Temperature Transmitter had not been inspected or tested since installation. Suncor Energy Inc did not inspect or test the Resistance Temperature Detector or Temperature Transmitter according to its mechanical integrity program for CAS testing nor the manufacturer's recommendations. Suncor Energy Inc, 5801 Brighton Boulevard, Commerce City, CO, was previously cited for a violation of this Occupational Safety and Health Administration Standard, 29 CFR 1910.119(j)(4)(iii), which was contained in Item 1 of Citation 2 issued on March 5, 2009 (OSHA Inspection number 311907711).
