29 CFR 1910.134(h)(3)(iv)(A): Respirators maintained for emergency use were not certified with the date the inspection was performed, the name (or signature) of the person who made the inspection, the findings, required remedial action, and a serial number or other means of identifying the inspected respirator: a) Throughout facility - The employer did not certify the inspections of Scott Pressure-PAK 2.2 SCBAs, used for emergency purposes, by documenting the date of the inspections, the name of the inspector, the findings, any required remedial action and an identification number for the SCBA, on or about 2/8/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
