29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. (a) On or about September 2, 2025, the employer had not maintained walking-working surfaces free from slip, trip, and fall hazards. Employees performing food preparation and cooking duties were working on a floor with loose, broken, and missing flooring tiles. Mitra Midwest Operations, LLC d.b.a. Kentucky Fried Chicken was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(3), which was contained in OSHA inspection number 1662226, citation number 01, item number 01, and was affirmed as a final order on September 21, 2023, with respect to the workplace located at 4702 NW Cache Rd, Lawton, OK 73505.
