29 CFR 1904.29(a):The employer did not use an OSHA 300, 301 and 300A Form or equivalent for recordable injuries and illnesses: a) South Unit Area Hallway - On or about June 25, 2025, an employee was injured due to tripping on buckling up loose floorboards when walking in the hallway checking on patients. The employee was out of work for 2 days returning to work on 6/28/2025. The employer did not keep OSHA 300 Logs. b) Hallway near Resident Dining Room - On or about August 31, 2025, an employee walking in the hallway going from the Resident Dining Room to the Supervisor's Office caught their foot on the buckling up loose floorboards causing the employee to trip and fall resulting in an in-patient hospitalization. The employee was still out of work on September 9, 2025. The employer did not keep OSHA 300 Logs.
