29 CFR 1910.147(c)(6)(ii): The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment, inspection date, employees included, and the person performing the inspection. a) On or about September 5, 2025, employees were exposed to hazardous energy, when the employer failed to ensure that periodic inspections of energy control procedures were properly documented, in which did not identify the machine or equipment involved, the date of the inspection, the names of authorized employees participating in the inspection, or the name of the individual who conducted the inspection, as required.
