29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer does not protect each employee from fall hazards while climbing onto the landing zone to perform roofing activities on residential homes and or structure at heights of approximately 22 feet above the ground This most recently occurred on July 11, 2025, during the installation of roofing material at a job site located at 4793 Dublin Road, North Royalton, OH 44133; the employer did not ensure employees were protected from a fall by extending the portable ladders 3 feet above landing zones. SKWINKLES HOME IMPROVEMENT, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(B)(1), which was contained in OSHA inspection number 1559388, citation number 02, item number 001 and was affirmed as a final order on (3/3/2022), with respect to a workplace located at 1031 Minuteman Ave NW, Massillon, OH 44646.
