Text For Citation: 02 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling from a crane suspended personnel platform: a. At pier E1, Mayport Naval Station, on August 14, 1991, warning or limiting devices were not installed on the P & H model 9125 truck crane, serial number 33632, to prevent two-blocking where audible communication had not been provided nor had any of the persons being lifted been specifically assigned the task of warning of the approach of a two-block condition. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install an anti two-blocking device as required by ANSI/ASME B30.5-1982, Section 5-3.2.2(a)(8).
