29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D). On or about 25 June 2025, employees of Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, were exposed to mechanical, atmospheric, and thermal hazards while working on or inside an industrial oven, Sugar Coat 1 Oven, due to inadequate energy procedures. a. A specific statement of the intended use of the procedure; b. Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; c. Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and d. Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. Gilster-Mary Lee Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1650556, citation number 1, item number 1 and was affirmed as a final order on 25 August 2023, with respect to a workplace located at 2301 S Wabash Avenue, Centralia, IL 62801. Per ISA - Remove Repeat designation & amend to Serious
