29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: Residential Sewer Connection Project, 7 Hamilton Court, Norwich, CT: On June 13, 2025, the employer, as the competent person, did not ensure the trench and adjacent areas were inspected prior to the start of work and as needed during the work, exposing the employee to trench and excavation hazards, resulting in fatal injuries.
