Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Alexander Hernandez DBA A&A Roofing & Siding LLC does not require the use of fall protection during roofing operations. This violation was most recently observed on June 2, 2025, at a commercial re-roofing project located at 2580 West 9th Avenue in Oshkosh, Wisconsin. Employees were exposed to falls hazards of 13 feet and greater while tearing off old roofing materials and installing new asphalt shingles without their harnesses being connected to lifelines or anchors.
