Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about June 2, 2025, at an establishment located at 2580 West 9th Avenue in Oshkosh, Wisconsin, employees of Octavio Saucedo dba Octavio's Home Improvement LLC were performing commercial roofing activities with no form of conventional fall protection while exposed to fall hazards of over 11 feet 3 inches. Octavio's Home Improvement LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1772020, citation number 2, item number 1 and was affirmed as a final order on January 21, 2025, with respect to a workplace located at 2135 E Belmont Place Oak Creek, WI 53154. Octavio's Home Improvement LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1687513, citation number 1, item number 1 and was affirmed as a final order on October 20, 2023, with respect to a workplace located at 1055 Huron Way, Hartford, WI 53027. Octavio's Home Improvement LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1669894, citation number 1, item number 1 and was affirmed as a final order on June 27, 2023, with respect to a workplace located at 303 Sunset Dr, Rio, WI 53960.
